For your ClinicMonk setup, you must follow these specific formatting rules for any form that collects a phone number.

You must add a checkbox to your form. To meet compliance, this checkbox cannot be pre-selected (the client must click it themselves).

Copy and paste this exact message next to your checkbox, replacing the bracketed info with your Legal Business Name:

"I consent to receive non-marketing text messages from [Legal Clinic Name] regarding appointment reminders, service updates, and clinical notifications at the phone number provided. Message frequency may vary. Message & data rates may apply. Text HELP for assistance, reply STOP to opt out."

Note: The name used above must match the Legal Business Name provided in your ClinicMonk A2P Brand Registration.

2. Strict Rules for Form Submission

  1. Consent is Optional: Even if you make the "Phone Number" a required field on your form, checking the SMS consent box must remain optional. You cannot prevent a client from submitting a form just because they didn't want to receive text messages.
  2. Separate Decision: The decision to provide a phone number and the decision to receive texts must be treated as two separate actions.
  3. No Pre-Checked Boxes: Carriers will automatically reject your application if they see a checkbox that is "Checked" by default.
